Referrer Update: Cornerstone Counselling

Cornerstone Youth Counselling Program (CYC) & Connected Care Update
Cornerstone Youth Services continues to deliver the Cornerstone Youth Counselling Program program funded by Primary Health Tasmania through the Australian Government’s Primary Health Networks Program. We have proudly delivered this program to young people aged 12-25 for the past 13 years covering the North and North West of Tasmania.
There is currently no change to our existing service, referral pathways or eligibility, and we continue to accept new referrals, however, Primary Health Tasmania is in the process of commissioning a new service called Connected Care – Mental Health, to improve coordination and integration between services and support people to move more easily between levels of care as their needs change.
The new service will replace a number of the low, moderate and high intensity services Primary Health Tasmania currently commission. Transition of the to the new model will start in July 2026 and will end by 31 January 2027.
Primary Health Tasmania has advised there will be a strong focus on continuity of care and maintaining referrals into existing programs while the new service is established.
More information will be provided progressively during the process of transitioning from the existing CYC program to the new service. Any changes will be communicated when they are known.

From our CEO

Cornerstone Youth Services has been part of the local Community in Launceston for over 30 years, previously known as ‘The Corner’. We now provide mental health services for young people from 12-25 years of age across the North and North West of Tasmania, via multiple programs.

Our foundation is based on four key values: Quality, Integrity, Contemporary and Inclusivity. We are passionate about youth health and our goal is to support young people to reach their full, and amazing, potential. Our incredible teams are our greatest asset, and we acknowledge this through creating an inclusive, supportive and positive work environment.

We value the importance of a great work culture.  This allows us to do the most important work, to support all young Tasmanians to live great lives.
